We are here to give you the tools necessary to take control of your Asthma. Education is the first step in understanding and managing your lung diagnosis. Asthma education is prescribed through your primary physician and it is available to patients with a diagnosis of Asthma and their family members.

Our program includes:

• Basic facts about Asthma inhaler or nebulizer technique instruction
• Reinforcement of Peak Flow monitoring
• Designation of Green, Yellow & Red zones on Asthma Action Plan
• Asthma Action Plan will be written, discussed and documented in the PT’s medical record
• Environmental control measures
• Emphasis of need of regular follow-up visits
• Receommend navigation of healthcare at Lakewood Health System
• Pre-test and post-test to measure outcomes
• Follow-up phone call from Asthma staff
