Social Services
Social Services provide a variety of services for hospital, hospice and Care Center patients. They work as an advocate for patients and are available to meet with patients and their families to discuss any questions or concerns they may have.
They provide discharge planning for patients in need of post-hospital services. Examples of discharge planning needs may include: home health care services, meal assistance, financial assistance, home medical equipment, etc.
Swing beds, transitional and Care Center beds are also services available at Lakewood Health System. Social Services assist in the assessment and admission of these patients. Swing beds and transitional beds are utilized for patients needing extended care services that meet appropriate insurance criteria. Care Center beds can include skilled nursing care or long term care services.
